We are using an online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email your registration form to us.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Please return your completed form to our Reception Team who will book an appointment for a health check appointment with our Healthcare Assistant. Once you have become registered with the Practice you can choose to see any Doctor.

We only accept patients onto our list if they reside within our Practice boundary, the Practice covers most BD17 postcodes and some outlying areas - please click on the blue CATCHMENT AREA CHECKER link below and type in your postcode to check if you are within the boundary area. Alternatively, please contact our Reception Team who will check for you.

All patients have been allocated a named GP which is a new requirement from NHS England. This does not alter how your care is delivered or who you see which remains entirely up to the individual.
If you would like to know who your named GP is, please ask our Reception Team or the GP you have come to see.
